AHG Annual Summit - 2024 Registration

In completing registration for this event, I hereby grant Articularis Healthcare Group and its licensees and assigns (collectively, “AHG”) the irrevocable right and permission to take and/or use photographs and/or video recordings of me, and to use my name, voice, likeness and persona, by all means and in all media now and hereafter known, throughout the world and in perpetuity, including without limitation on any AHG or other websites and in publications; advertising, promotional, educational and other materials; to make derivative works thereof; and for any other similar purpose without compensation to me. Without limiting the foregoing, I further irrevocably give and grant to AHG the unlimited right to adapt, change, edit, modify, add to, delete, reproduce, distribute and otherwise exploit same in any manner whatsoever without my review or approval.

I understand and agree that such photographs and/or video recordings of me may be placed on the Internet. I also understand and agree that I may be identified by name and/or title in printed, Internet or broadcast information that might accompany the photographs and/or video recordings of me. I waive the right to approve the final product. I agree that all such portraits, pictures, photographs, video and audio recordings, and any reproductions thereof, and all plates, negatives, recording tape and digital files are and shall remain the property of AHG.

I hereby irrevocably release, acquit, and forever discharge AHG, and their agents, representatives, officers, and employees, from any and all claims, demands, rights, promises, damages and liabilities arising out of or in connection with the exercise of the granted rights or use or distribution of the information and materials described in this release, including but not limited to any claims for invasion of privacy, appropriation of publicity rights or likeness, or defamation.

I warrant that I am the age of majority or older where I reside and have fully read, understand, and agree to this release.
